WORK INDEPENDENTLY · 1-to-1 IN-HOME PATIENT CARE

RESPONSIBILITIES:

Perform home visits for patients as delegated by the care managing RN
Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
Supports the patient and family as appropriate to the patient’s condition

QUALIFICATIONS:

Current certification as a CNA in Connecticut
Valid driver’s license
Reliable transportation and willing to travel in New London County
Strong communication and interpersonal skills
Home health experience preferred

Compensation: $21.00 - $25.00 per hour
