RESPONSIBILITIES:
- Assess, evaluate, and provide care for patients in a home care setting.
- Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
- Teach supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
QUALIFICATIONS:
- Current license as a Physical Therapist in Connecticut
- Valid driver’s license
- Reliable transportation and willing to travel in a designated area that may include New London, Windham, and Middlesex counties
- Strong communication and interpersonal skills
- Home health experience including Medicare preferred

Compensation: $65.00 - $110.00 per hour
